Fees and Insurance

Payment for services rendered is due at the time of service.  

●      This includes copays and/or insurance deductible/patient portion deposits, and any balance owed. 

●      Regardless of insurance coverage, the client is always responsible for their fees.

●      Accepted forms of payments are Credit and Debit Cards.

○      (Cash and Check is not accepted)

●      Accepted Insurances include Aetna, AllKids, Blue Cross Blue Shield of AL., Medicaid (AL), and Medicare (AL).

Cancellation Policy
If you cancel or do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, a $50.00 out-of-pocket fee will be charged to your account.

Court Appearance 1,500.00 (per day) (payment is due even if clinician does not testify)
